3px outline with 3px offset. Tab/Shift+Tab navigation; Enter/Space activation; Escape in overlays.
Errors use text and aria-invalid. Skeletons are hidden from readers; the container announces state.
Prefer 44px actions. Compact variants need extra spacing in touch interfaces.
Use Tab to verify visible focus.
